Assorted food in baine marie was held at 55 °F, in the prep area, rather than 41°F or below as required. Assorted food in the self serve case were held at 44-50°F.

The storage shelves and plastic dish washer rack used to hold cleaned food contact items at the dish sink are not clean to sight and touch, presenting a source of contamination of the cleaned items.

Ice machine is not cleaned at a frequency to prevent the presence of mold. Containers observed on drying shelf with glue residue and were not clean to sight and touch.
