On Saturday, March 14, from 9 a.m. to 1 p.m., a “Passport Acceptance Day” will be held in the prothonotary’s office in Room 104 of the Lebanon County Municipal Building at 400 South Eighth St.

No appointment is needed, but anyone applying for a passport must personally come to the office.

Passport Acceptance Day was a proposal by Clerk of Courts/Prothonotary Barbara Smith that had been approved for this year at a January meeting of the Lebanon County Commissioners.

To be able to apply for a passport, certain documents will be needed and must be brought with you. They include:

  • A passport application, which is available in the prothonotary’s office, and must be filled out in black ink, only.
  • An original birth certificate, from the state and with a raised seal, and both parents’ names must be on the birth certificate unless the father was not recorded, or a previously issued passport (expired for more than five years).
  • Naturalization certificate, in lieu of birth certificate.
  • Passport photo, which can be taken in the prothonotary’s office for $10.
  • Copy of driver’s license or photo identification card.

For children, they will need copies of both parents’ driver’s licenses or state ID, even when using the special consent form.

To get a passport book, the cost is $110 for ages 16 and older.

For ages 15 and under, the cost for a passport book is $80.

These are payable to the US Dept. of State, and payment may be in the form of a check or money order.

For the card, the cost is $30 for those age 16 and older, and $15 for those 15 and under.

There is a $35 processing fee for each application. This fee is payable to Prothonotary and may be paid by cash, check, or money order.

Processing fees may be combined.

Credit and/or debit cards cannot be accepted for this transaction.

Visitors should not sign the passport application until asked to do so by an agent.

Passports will be processed in six to eight weeks.
