The floor drain screens throughout the food facility was dirty and filthy and in need of cleaning. In the deli, the floor and the wall behind the oven were observed with a build-up of grease and dirt and were in need of cleaning. The flooring in all departments was observed with an accumulation of filth and was in need of cleaning.
6 – 501.14 Cleaning Ventilation Systems, Nuisance and Discharge Prohibition
Observed an accumulation of grease at the hood filters in the deli, which were in need of cleaning.
4 – 602.13 Non-Food Contact Surfaces
The fan guards throughout the food facility’s walk-in cooler were observed with an accumulation of dust and soil and were in need of cleaning. Observed the gasket of the cooler in the fruit cutting room with an accumulation of dirt and soil.
7 – 201.11 Separation-Storage
In the deli area, spray bottles of chemicals were observed hanging from the bar on the prep table, directed at the deli bags and food. In the produce cutting room, two bottles of sanitizer were stored on the fruit cutting table. Corrected.
6 – 201.11 Floors, Walls, and Ceilings-Cleanability
The floor grout in the deli department was low and needed to be replaced. The ceiling in the hallway of the dishroom and the prep area was leaking and needed to be repaired.

Assorted food contact items were observed over the sink with an oily residue and were not clean to the sight and touch. Removed for cleaning.

The paper towel dispenser was empty at the handwash sink in the rear of the prep area. A cloth towel was observed hanging on a nail at a shelf. It appeared that this towel was being used for employees to dry their hands.

Observed in-use knife and/or cleavers stored between table edges or between tables, areas not easily cleanable and sanitized.
4 – 101.11 Characteristics-Materials for Construction and Repair
Observed plastic containers used for food storage in a deteriorated condition. Discussed the condition of plastic containers with the owner. The plastic had deteriorated due to hot food. The deteriorated plastic cannot be properly cleaned. Numerous plastic containers were to be discarded.
3 – 304.11 Food Contact with Equipment, Utensils, and Linens
Cardboard was observed being used in contact with food; it is not possible to properly clean and sanitize cardboard.
6 – 501.114 Maintaining Premises, Unnecessary Items, and Litter
Two broken food tongs were stored in a bucket of cleaning supplies. Discarded.
3 – 305.11 Food Storage, Preventing Contamination from the Premises
Observed that the underside of a shelf above the Bain Marie and hot food-holding equipment were not clean. Observed cardboard being used to hold cooked egg rolls; cardboard is not a suitable material for food contact.
3 – 501.14 Cooling
Observed egg rolls being cooled on three large piles on a metal tray and on cardboard. The egg rolls in the center had a higher temperature than the egg rolls on the exterior. The facility was not monitoring the temperature and time to ensure that this food item was cooled from 135°F to 70°F in two hours and/or from 135°F to 41°F in six hours after cooking.
4 – 501.12 Cutting Surfaces
Observed three deeply scored cutting boards not resurfaced or discarded as required. Discarded.
4 – 601.11(A) Equipment, Food Contact Surfaces, Non-Food Contact Surfaces, and Utensils
Food containers and equipment were observed to have food residue and were not clean to the sight and touch.
4 – 602.13 Non-Food Contact Surfaces
Non-food contact surfaces were not cleaned at a frequency to preclude the accumulation of dirt and soil. Specific examples were the bottom shelf of a prep table and the shelves in the walk-in cooler.
7 – 102.11 Common Name-Working Containers
One spray bottle, a working container in the prep area, was used for storing *chemicals, cleaners* taken from bulk supplies and was not marked with the common name of the chemical.
