Dr. Thomas A. “Tom” Shaak, 90, of Lebanon, died on Thursday, Dec. 24, 2020, at Middletown Nursing Home.

He was born on Saturday, Feb. 8, 1930, to the late Lee Shaak and Carrie Shaak nee Swanger in Lebanon. He was a veteran of the Air Force in the Korean War. Tom was a dentist for over 40 years in Lebanon and enjoyed fishing, traveling, and spending time with his family.

Surviving is children, Cathy L. Wise, and spouse, Frank, Cynthia K. Emma, and spouse, Leonard, Caren A. Butera, and spouse, Joe, Thomas L. Shaak; seven grandchildren; four great-grandchildren; and brothers, George D. Shaak and John J. Shaak. He was preceded in death by his wife, Mary A. Shaak nee Swartz; and siblings, James F. Shaak and Bernard L. Shaak.

Services will be at the convenience of the family.
