2 – 402.11 Effectiveness-Hair Restraints
Food employees observed in prep area wearing proper hair restraint, but not in a manner that controls all hair(pony tail). Other employee observed not wearing beard net or mask that covers beard.

3 – 501.18 Ready-to-Eat, Time/Temperature Control for Safety Food, Dispostion
Tuna salad with egg, chicken salad, chicken noodle soup, chicken in BBQ sauce, vegetable beef soup, refrigerated ready to eat time/temperature control for safety food, in the walk in cooler , was not compliant with date marking by being labeled with a discard or use by dates of up to 4 months prior to the date inspection and were not discarded.

3 – 701.11 Discarding or Reconditioning Unsafe, Adulterated or Contaminated Food
Assorted prepared and commercially packaged foods were adulterated by prolonged temperature abuse and the owner/operator did not discard as required by the PA Food Code after being directly instructed during the inspection. Instead, the hazardous items well left on display and sandwiches made from abused salads were either again made and placed for sale or returned for sale after having been removed and ordered destroyed.

The Person in Charge does not have adequate knowledge of food safety in this food facility as evidenced by this non-compliant inspection.

4 – 602.13 Nonfood-Contact Surfaces
Non-food contact surfaces, such as shelves, knobs and handles, not cleaned at a frequency to preclude accumulation of dirt and soil.

3 – 101.11 Safe, Unadulterated & Honestly Presented
After being ordered to remove and discard temperature control for safety food in the walk in cooler that was temperature abused, the owner/operator placed the same food for sale after the initial inspection. This condition was observed as a follow up after leaving the facility for 1 hour. The sandwiches, in place of those that were removed, were at 55F. In addition, other unsafe foods were on display for sale at 56-57F, including milk, eggs, meats, and cheeses.

3 – 501.16(A1) Time/Temperature Control for Safety Food, Hot Holding
Hot dogs and sausages were held at 114-125 °F, on the roller grill, rather than 135°F or above as required.

4 – 703.11 Hot Water and Chemicals-Methods
Ice machine not cleaned at a frequency to prevent the presence of mold. The unsanitary condition and items left in sanitizer basin of the 3 bay sink indicates that food contact surfaces and utensils are not being immersed for a minimum of 30 seconds in the quat sanitizing compartment of the manual ware washing sink.

6 – 501.12 Cleaning, Frequency and Restrictions
Litter, debris, and residue under and around equipment in prep and storage areas.

3 – 403.11 Reheating for Hot Holding
Hot dogs and sausages were not reheated to 165°F within two hours, for hot holding. The temperatures ranged from 114-125°F within 2hrs of having been put on the roller grill.

3 – 501.16(A2)(B) Time/Temperature Control for Safety Food, Cold Holding
All food in the walk in cooler was held at 45-50°F, rather than 41°F or below as required.

4 – 501.14 Warewashing Equipment, Cleaning Frequency
3 bay sink observed with build up of filth and food residue and not cleaned before use, and frequently throughout the day. Condition of old filth was present after food handler had washed items just prior to this inspection.

4 – 901.11 Equipment & Utensils, Air Drying Required
Plastic food containers observed drying on dirty cloth towels and not a clean drying rack that allows proper air flow for drying.

Food facility does not employ a certified food employee as required.
