Cardboard cannot be properly cleaned; if cardboard is going to be used to cover areas that accumulate grease or debris, the cardboard should be changed frequently as it becomes soiled. One stainless prep table has a plastic covering that is very deteriorated and cannot be properly cleaned. The plastic covering should be removed, and the table should be properly cleaned. Several non-food contact surfaces are not being cleaned at a frequency to preclude the accumulation of dirt and soil; specifically, the door to the employee toilet room is not clean, and the surface is difficult to clean; the fan covers in the walk-in cooler need to be cleaned.

The three fan covers on the ceiling of the meat-cutting room are extremely dirty and in need of cleaning.

Observed a wooden piece of furniture that was intended for changing a baby being used as a shelf for supplies in the meat area. Observed another shelf/cart that is extremely rusted. Several cardboard boxes are being used for storing ingredients and other items in the meat room/meat and deli area. Cardboard cannot be cleaned, and if it is going to be used, it should be changed frequently.

The interior of the ice machine, a food contact surface, was observed to have brown residue and was not clean to sight and touch.

Non-food contact surfaces were not cleaned at a frequency to preclude the accumulation of dirt and soil; specifically, food debris was observed under the slicer, on the floor behind the slicer prep table, and on the cords for the slicer and other equipment. Paper and foil covering on the shelves of the produce section has deteriorated and cannot be properly cleaned.

One glass door on the meat display cases is broken and covered with cardboard. The cove molding in the deli/meat display case area has deteriorated and is falling off of the wall. The material along the floor/wall junction behind the prep table with the meat grinder is also in poor condition.

The interior of a cabinet used for ingredients and equipment in the kitchen has old food debris and other items that should be removed. Two containers had food debris in the bottom of them. These items were cleaned immediately.

The perimeter of the produce cutting room has a significant amount of debris and items that should be removed particularly under the produce prep table.
