The bottom of the slicer was observed covered with plastic and foil. Plastic and foil cannot be cleaned. This part of the slicer should be covered with a material that is durable and can be cleaned and sanitized as needed.

Thermometer for ensuring the proper temperature of food was not available or readily accessible.

One freezer was plugged into an extension cord instead of being plugged directly into an outlet as required by City Ordinance.

The screen door located in the rear area of the food facility had a gap and did not protect against the entry of insects, rodents, and other animals. The door was broken and needed to be replaced.

Soap was not available at the handwash sink in the prep area. Soap was available at the sink by the food service area, but there was a problem with the hot water faucet at that sink.

Observed food prep equipment and exposed bakery items in the food storage area while the overhead doors were open. This is a violation of plans that had been submitted to the City of Lebanon in order to get a Certificate of Occupancy.

The handwashing sink by the hot food service area was removed when the area was modified for alcohol sales.

Lights in the hot food prep area and lights over the stoves were not functioning.

A problem with the meat band saw was “fixed” by using plastic made into a string to tie parts together. Plastic “string” cannot be properly cleaned and sanitized.

Foaming soap was stored on the same shelf as snack food items with no barriers. Corrected. No rinse wipes were stored on a shelf above snack foods in the backroom area. Corrected.
