The can-opener blade was observed to have a build-up of dry food and was not clean to the sight and touch. Corrected. The interior top of the microwave was observed with an accumulation of old food debris on the surface. Corrected.

Observed clean food utensils in the prep area, stored wet in a manner that did not allow for draining and/or air drying (wet nesting). Observed clean food utensils in the pot washing area, stored not inverted.

The food facility inspection indicated evidence of rodent activity (droppings and paper gnawing in storage, laundry, and employee bathroom area), but the facility had a pest control program. The areas were cleaned during the inspection, and the pest control reports were reviewed.

The wall in the dry storage area was made of drywall and was cracked/roughened/and was not a smooth, easily cleanable surface. The ceiling tiles in the kitchen prep area were observed to be saggy, stained, and in need of cleaning.

The floor and the walls around the mop sink were deteriorating and no longer able to be cleaned. This scenario represented a critical sanitation and safety failure in a food service or commercial environment. Deteriorated surfaces around a mop sink are a known hazard because constant moisture encourages the growth of pathogens—such as Listeria monocytogenes, E. coli, Salmonella, and Legionella—and provides a breeding ground for mold and mildew.

Observed chopped vegetables being cooled in the refrigerator/walk-in cooler in containers with tight-fitting lids, which is not a proper cooling method. The product was made at a time that allowed for breaking it down for rapid chilling within two hours of processing.

Non-food contact surfaces were not cleaned at a frequency to preclude the accumulation of dirt and soil.

All food in the walk-in cooler was held at 52-59°F, rather than 41°F or below as required. All processed items were discarded, and the whole fruit and produce items were moved to another cooler.

The plumbing system was not maintained in good repair. Observed the dishwasher on the third floor leaking, the uncapped, unused drain pipe behind bard on the second floor, and water leaking under the dishwasher in the first-floor bar.
