Several plastic food containers made of hard plastic had broken and missing pieces. Some had broken corners on three of the four corners.

Exposed food preparation observed in area under dirty ceiling light fixture, and subject to potential contamination.

The sides of the rolling racks used for trays of product are not clean. One food ingredient container that was on the rack of clean food containers, but had been stored wet was covered with black mold. The food container used for dry ingredients and one used for parmesan cheese, both food contact surfaces, were observed to have food residue and were not clean to sight and touch.

Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil; specifically, the paper towel dispenser handle and the soap dispenser at the rear handwashing sink and the walk-in cooler door handle. There was a hair stuck in the dirt on the soap dispenser.

The food contact surfaces on several plastic food containers are not smooth, easily cleanable and/or resistant to pitting, cracking or scratching. The items can no longer be properly cleaned and should be discarded.

Old unused hard-plastic equipment stored in the area above the walk-in cooler has broken and missing corners and should be removed from food facility.
