Food facility has a Pest Control Program, but the program is not being monitored properly by personnel at the facility. Several of the interior traps appear that they are not being checked each month. Documents in the PCO’s log book are not current. The Applicator licenses expired 2/28/21. The PCO’s business license from the Commonwealth of Pennsylvania expired 12/31/21. The insurance certificate expired 12/31/21. Additionally, a finding on reports had not been addressed by the facility for several months.

Non-food contact surfaces not cleaned at a frequency to preclude accumulation of dirt and soil. Specific areas noted are behind a proofing case in the bakery, behind the fryer in the chicken prep area, a shelf above the 3-compartment sink in the produce department and the fans in the dairy walk-in cooler.
