The Food Facility does have a Pest Control Program; however, it was observed that Pest Control Operator (PCO) had not checked every trap at the contracted frequency. The owner of the facility also reported that the PCO has not supplied required documents such as chemical labels, Safety Data Sheets, copies of licenses, etc.
