The PCO not changing glue boards for more than 600 days and the management not conducting self-inspections so that a deficiency like this could be observed is not an effective Pest Control Program. A door gap was noted in the PCO’s findings report from 10/3/22 until 9/18/23; it now shows “complete,” but there is a new finding showing the same issue. The PCO is not providing specific details so that the management can know what to repair. The facility management is not reviewing the findings in the reports and asking the PCO for more specificity.

For example, the notes mention a “gap under a door.” The report should specify exactly which door so that management can correct the deficiency. “Unable to inspect all areas” is another example where the PCO should work with facility management to identify and correct the problem.
