Footer of the food slicer is in need of replacing teeth are worn down and broken off. Bain Marie at the pizza station lid is repaired with duct tape, which is unapproved material for repair. Corrected. In the stand-up one-door freezer, raw wooden shelves are being used, which is not an approved material.

Food slicer, a food contact surface, was observed to have food residue and was not clean to sight and touch. Corrected.

Food employees observed in pizza making and grill area not wearing proper hair restraints, such as nets or hat covers.
3 – 501.19 Time as a Public Health Control
Time in lieu of temperature being used in the food facility to control ready-to-eat potentially hazardous foods (cooked pizza) without written procedures or documentation to verify disposition of food. This is a repeat violation. Corrected.
4 – 501.11 Good Repair and Proper Adjustment-Equipment
Door gaskets on the three-door reach-in cooler are severely damaged and in need of replacing. Fan guard cover missing on the two-door upright reach-in, in the storage area.

Ice chute at the self-service fountain area with pink slim-like matter accumulation and not clean to sight and touch. Corrected.
6 – 101.11 Surface Characteristics-Indoor Areas
Flooring throughout the facility has chipping epoxy and is in need of repair. Wall beside food slicer table with paneling breaking and chipping away and needs to be repaired.
6 – 202.15 Outer Openings, Protected
Exterior door located in the storage shed of the food facility has a gap and does not protect against the entry of insects, rodents, and other animals. Door needs to replaced or repaired.
