Soup ladles, food contact surfaces, were observed to have food residue and were not clean to the sight and touch. Corrected, moved to dirty dish.

The food contact surfaces of equipment and utensils did not reach 160°F during the sanitizing cycle of the high-temperature dishwasher, as evidenced by the failure of the temperature indicator. Inspector’s digital min-max thermometer 152°F, facility disc thermometer 146°F. Booster heater was turned on dial thermometer on the outside of the machine reading 190°F. Food facility will use the three-bay sink.

4 – 903.11 (A-B)(D) Equipment, Utensils, and Linens, Storing
Observed clean dish racks stored on the floor in the dishwashing area. Observed clean food equipment and/or utensils in the clean dish area stored wet in a manner that did not allow for draining and/or air drying (wet nesting).

6 – 301.12 Hand Drying Provision
Paper towel dispenser was empty at the handwash sink in the dish area. Corrected.

6 – 501.111 Controlling Pests
Food facility inspection indicated evidence of rodent activity (droppings) by the ice machine, boiler, hot water heater, storage room, and serving line in the cabinet on hot holding line areas, but the facility did not have a pest control program.

Due to a customer complaint, three toilet rooms were inspected at this facility. Toilet tissue was not on the dispenser in any of the toilet rooms and therefore was not being dispensed in a sanitary manner. When toilet tissue is not securely mounted on a dispenser, users are forced to handle the exposed roll directly. This defeats the primary purpose of a dispenser, which is designed to provide sanitary, touch-free access to only the sheet being used, thereby preventing cross-contamination.
