Bethel Township

Vehicle Accident – On June 25, 2021 at 3:46 a.m., this collision occurred as Unit 1, a 2019 Peterbilt Truck Tractor, driven by Operator 1, a 65 year old Clermont, FL man, and Unit 2, a 2015 Freightliner Truck Tractor, driven by Operator 2, a 40 year old Douglasville, GA man, were parked on the shoulder for the I 78 offramp on exit 6, when Unit 1 drifted back and struck Unit 2. Both vehicles came to rest on the shoulder, facing east. Operator 1 and Operator 2 did not report injuries as a result of this crash.

City of Lebanon

DUI – On June 13, 2021 at 3:33 a.m., at Willow St. and North Cherry St., a traffic stop conducted by members of PSP Jonestown found the vehicle Operator to be under the influence of alcohol. Charges for this incident have been filed through Magisterial District Court 52-2-01. The Arrestee was a 26 year old Lebanon woman. The vehicles involved included a 2009 Nissan Altima.

DUI – On June 28, 2021 at 2:16 a.m., at West Cumberland St. and Dale St., a traffic stop conducted by members of PSP Jonestown found the vehicle Operator to be under the influence of alcohol. Charges for this incident are pending forensic blood analysis results. The vehicles involved included a 2003 Ford Explorer.

Cornwall Borough

Deer Struck – A 25 year old Lebanon resident struck a deer on June 24, 2021 at 9:11 p.m. on Boyd St. at Gatehouse Ln. No injuries were reported as a result of this crash. The vehicle sustained minor damage but was drivable.

Deer Struck – A 42 year old Lebanon resident struck a deer on June 24, 2021 at 9:14 p.m. on SR 322 at Boyd St. No injuries were reported as a result of this crash. The vehicle was towed from the scene.

East Hanover Township

Vehicle Accident – On June 27, 2021 at 3:18 p.m., this two-vehicle crash occurred on I 81 Northbound near mile marker 83.2 in East Hanover Twp. This two-vehicle crash occurred as Unit 2, a 2018 Nissan Murano, driven by Operator 2, a 56 year old Pittston woman, was slowing for traffic in front of her. Unit 1, a 2010 Honda CRV, driven by Operator 1, a 24 year old Myerstown man, failed to slow and subsequently rear-ended Unit 2. Both vehicles sustained disabling damage and were towed from the scene by Motters Towing. Operator 1 was not injured as a result of this crash. Operator 2 sustained a small bruise on her leg from impact with the dash but refused medical treatment. Operator 1 was cited for Driving Vehicle at Safe Speed.

Vehicle Accident – On June 28, 2021 at 9:14 a.m., this crash occurred on Allentown Blvd. eastbound at its intersection with Harrison School Rd. in East Hanover Twp. Unit 1, a 2011 Toyota Sienna, driven by Operator 1, a 42 year old Annville man, was traveling southbound on Harrison School Rd., preparing to cross Allentown Blvd. Unit 2, a 2020 Nissan Rogue, driven by Operator 2, a 67 year old Elizabethtown woman, was traveling east on Allentown Blvd. Unit 1 was stuck by Unit 2 in the middle of the intersection. Both Unit 1 and Unit 2 came to final rest in a field on the south side of Allentown Blvd. Both Unit 1 and Unit 2 sustained disabling damage and were towed from the scene by H & S. Operator 1 sustained no injuries as a result of this crash. Operator 2 sustained suspected minor injuries as a result of this crash and was transported via Life Lion to the Hershey Medical Center. PSP Jonestown was assisted on scene by members of PSP K-9, Fort Indiantown Gap fire department, and Life Lion EMS. Operator 1 was cited for Stop Signs and Yield Signs.

Identity Theft – On June 30, 2021 at 4:09 p.m., PSP Jonestown responded to Saw Mill Ln. for a report of an individual receiving unemployment information from her employer. The Victim related that she never filed for unemployment. PSP is investigating. The Victim was a 56 year old Grantville woman.

Heidelberg Township

Identity Theft – On June 29, 2021 at 5:02 p.m., PSP Jonestown responded to Sunnyside Rd. for a report of an individual receiving unemployment information from his employer. The Victim related that he never filed for unemployment. The Victim was a 49 year old Newmanstown man. PSP is investigating.

Accidental Death – On July 3, 2021 at approximately 9:46 a.m., troopers from PSP Troop L responded to Sinclair Rd. for a report of a child having been struck by a vehicle. Upon arrival, troopers found the 2 year old juvenile male victim deceased in the driveway of the residence. Through investigation, it was learned that the victim’s father, believing that all children were out of the vehicle and with his wife, moved the SUV forward. While moving the vehicle forward, the vehicle struck the victim, causing fatal injuries. The death is believed to be accidental; however, the investigation continues. PSP Troop L was assisted on scene by Schaefferstown Fire and EMS and the Lebanon County Coroner’s Office. The victim was a 2 year old Myerstown juvenile male. The vehicles involved included a 2012 Chevrolet Suburban.

Identity Theft – On July 1, 2021 at approximately 6:10 p.m., PSP Jonestown responded to Glassmoyer Ln. for a report of a theft. The Victim was notified by her employer via mail that an unemployment claim was opened, utilizing her social security number, on July 1, 2021 at approximately 12:00 a.m. The Victim contacted the Department of Labor and Industry and completed the proper fraudulent forms. The Victim also notified her personal bank who advised she did not sustain any monetary losses and would monitor any suspicious activity. On July 1, 2021 at approximately 6:40 p.m., Syreeta Scott, U.S. Department of Labor was notified of this incident. Investigation is ongoing. The Victim was a 62 year old Newmanstown woman. The stolen property included a Social Security Number.

Vehicle Accident – On June 30, 2021 at 7:40 a.m., this crash occurred on SR 501 at its intersection with Reistville Rd. in Heidelberg Twp. Unit 1, a 2018 Chevrolet Impala, driven by Operator 1, a 65 year old Lebanon man, was traveling west on Reistville Rd., stopped at a properly posted stop sign. Unit 2, a 2014 Nissan Titan, driven by Operator 2, a 40 year old Myerstown man, was traveling south on SR 501. Unit 1 proceeded without clearance after stopping. Unit 1 was stuck in the intersection by Unit 2. Both Unit 1 and Unit 2 came to final rest in the yard of the residence on the 00 block of West Reistville Rd. Both Unit 1 and Unit 2 sustained disabling damage and were towed from the scene by CJ Wagner’s. Both Operator 1 and Operator 2 reported suspected minor injuries as a result of this crash. PSP Jonestown was assisted on scene by the Lebanon County fire companies. Operator 1 was cited for Stop Signs and Yield Signs.

Jackson Township

Vehicle Accident – On June 28, 2021 at 3:31 p.m., this reportable crash occurred as Unit 1, a 2009 Kia Rondo, driven by Operator 1, an 84 year old Womelsdorf man, was attempting to turn left onto East Lincoln Ave. (SR 422) and Unit 2, a 2006 Nissan X-Terra, driven by Operator 2, a 26 year old Lebanon woman, was traveling west on SR 422 in Jackson Twp. Operator 1 failed to yield to oncoming traffic and attempted to turn left in front of Unit 2, causing an angle collision. Initial impact occurred as the front bumper of Unit 2 struck the left rear side of Unit 1. Unit 1 came to final rest, facing north off of the shoulder of SR 422 and Martin Rd. Unit 2 came to final rest, facing west on the shoulder of SR 422. Operator 1 reported chest injuries of unknown severity and was transported via Myerstown First Aid Unit to the Good Samaritan Hospital. Operator 2 was evaluated on scene by EMS and released. Operator 1 was cited for Stop Signs and Yield Signs.

Identity Theft – On June 29, 2021 at 5:00 p.m., PSP Jonestown responded to Laurel Dr. for a report of an individual receiving unemployment information from his employer. The Victim related that he never filed for unemployment. The Victim was a 35 year old Myerstown man. PSP is investigating.

Identity Theft – This incident occurred on an unknown date and unknown time as unknown Actor(s) used the identity of the Victim to file for unemployment through the PA Department of Labor and Industry. The Victim related that she did not file for unemployment. The Victim related that she received an email from her employer regarding her filing of unemployment on July 1, 2021. The Victim related no financial loss from this incident. The Victim related that she filed a fraudulent claim report on uc.pa.gov website. Notification was made with U.S. Department of Labor regarding this incident. The Victim was a 44 year old Myerstown woman. The stolen property included Identity.

Identity Theft – This incident occurred on an unknown date and unknown time as unknown Actor(s) used the identity of the Victim to file for unemployment through the PA Department of Labor and Industry. The Victim related that she did not file for unemployment. The Victim related that she received a debit card for unemployment in the mail on July 1, 2021 and an email from her employer regarding her filing of unemployment on July 2, 2021. The Victim related no financial loss from this incident. The Victim related that she filed a fraudulent claim report on uc.pa.gov website. Notification was made with U.S. Department of Labor regarding this incident. The Victim was a 48 year old Myerstown woman. The stolen property included Unemployment Fraud.

Vehicle Accident – On June 30, 2021 at 10:10 a.m., this crash occurred on West Lincoln Ave. at its intersection with Locust St. in Jackson Twp. Unit 2, a 2015 Volkswagen Jetta, driven by Operator 2, a 47 year old Lebanon woman, was traveling east on West Lincoln Ave., stopped at a red light. Unit 1, a 2008 Toyota Tacoma, driven by Operator 1, a male, accompanied by Passenger 1, a male, was traveling directly behind Unit 2. Unit 1 rear-ended Unit 2. Operator 1 then fled from the scene of the crash in Unit 1. The extent of the damage to Unit 1 is unknown. Unit 2 sustained minor damage and was driven from the scene by Operator 2. It is unknown if Operator 1 or Passenger 1 sustained any injuries as a result of this crash. Operator 2 sustained no injuries as a result of this crash. Anybody with information pertaining to this crash is asked to contact PSP Jonestown and reference incident #PA2021-890149.

Identity Theft – On June 28, 2021 at 11:44 a.m., troopers responded to a report of identity theft at Kutztown Rd. The Victim reported that her personal information was used to file for unemployment benefits. Troopers are investigating. The Victim was a 43 year old Myerstown woman. The stolen property included Personal Information.

Myerstown Borough

Theft Of Vehicle Parts – Sometime between June 24, 2021 at 11:00 a.m. and June 25, 2021 at 9:00 a.m., troopers responded to a theft of a license plate from the back of a vehicle at West Main Ave. and North Goodwill St. Troopers are investigating. The Victim was a 26 year old Myerstown woman. The stolen property included a PA Registration Bearing LHF6989.

Theft Of Vehicle Parts – On June 28, 2021 at 11:51 a.m., troopers responded to West Lincoln Ave. for a report of a stolen catalytic converter from the underside of a vehicle. Upon investigation, it was discovered that the catalytic converter was cut off of the vehicle. Troopers are investigating. The Victim was a 71 year old Ephrata man. The stolen property included Part Honda Element Catalytic Converter.

Richland Borough

Identity Theft – This incident occurred on an unknown date and unknown times as unknown Actor(s) used the identity of the Victim to file for unemployment through the PA Department of Labor and Industry. The Victim related that she did not file for unemployment. The Victim related that she received a debit card in the mail and received an email from her employer regarding her filing of unemployment on July 1, 2021. The Victim related no financial loss from this incident. The Victim related that she did file a fraudulent claim report on the uc.pa.gov website. Notification was made with Syreeta Scott, U.S. Department of Labor regarding this incident. The Victim was a 42 year old Richland woman. The stolen property included Identity.

Identity Theft – On June 30, 2021 at 8:03 a.m., State Police responded to West Main St. for an identity theft. Through investigation, it was determined that an unknown individual utilized the Victim’s personal identifiable information (PII) to file a fraudulent claim for unemployment benefits. The Victim incurred no financial loss as a result of this incident. The Victim was a 40 year old Richland man. The stolen property included the Victim’s Identity.

South Londonderry Township

Accidental Discharge Of Firearm – On June 27, 2021 at 5:04 p.m., this incident occurred on the 100 block of Schoolhouse Rd. as a 68 year old male Subject suffered a gunshot wound after he had an accidental discharge when he was handling a firearm. No one else was injured as a result of this incident. The male, who was conscious and alert, was taken to the hospital for his injuries. South Londonderry Twp. police department, Campbelltown Fire and First Aid & Safety Patrol responded to the call.

Vehicle Accident – On June 30, 2021 at 9:37 p.m., a Subaru driven by an 18 year old Palmyra resident, struck a Lexus driven by a 35 year old Lebanon resident, in the 2800 block of Horseshoe Pk. The Lexus was stopped, waiting to make a left turn. No injuries were reported as a result of this crash, and both vehicles were driven from the scene.

Vehicle Accident – On June 25, 2021 at 4:44 p.m., a Ford truck driven by a 56 year old Elizabethtown resident, turned from Route 117 onto Route 322 into the path of a VW driven by a 19 year old Hershey resident. The Hershey resident was facing a green light and had the right-of-way. No injuries were reported as a result of this crash, and both vehicles were able to be driven from the scene.

Swatara Township

Disorderly Conduct – On July 2, 2021 at 12:19 a.m., PSP Jonestown troopers responded to the 00 block of North Poplar St. for a reported disorderly conduct incident. Upon arrival, troopers spoke with the Victim, who related that the Arrestee drove past her house and shouted at her. The Arrestee admitted to doing the same and was cited, via non-traffic citation, for Disorderly Conduct. Charge is pending before MDJ Wolfe 52-3-03. The Arrestee was an 18 year old Lebanon man. The Victim was a 37 year old Lebanon woman. The vehicles involved included a 2003 Ford Focus.

Theft Of Vehicle Parts – Sometime between June 23, 2021 at 8:00 a.m. and June 24, 2021 at 6:45 p.m., troopers responded to Darkes Rd. for a report of a license plate. The Victim reported that the license plate was stolen from the back of his vehicle. Troopers are investigating. The Victim was a 58 year old Jonestown man. The stolen property included PA Registration LLZ6723.

Identity Theft – On June 22, 2021 at 11:00 a.m., troopers responded to a report of identity theft at North Mill St. The Victim reported that his personal information was used to apply for unemployment benefits. The investigation is ongoing. The Victim was a 46 year old Jonestown man. The stolen property included the Victim’s Personal Information Was Fraudulently Used.

Union Township

Vehicle Accident – On July 1, 2021 at 12:14 p.m., this crash occurred as Unit 1, a 2020 Kenworth Northwest T680, driven by Operator 1, a 67 year old Salisbury man, and Unit 2, a 2013 Peterbilt T386, driven by Operator 2, a 51 year old Bakersfield man, were traveling south on I 81 Southbound near mile marker 89.2. Unit 1 was traveling in the left lane. Unit 2 was traveling in the right lane, just ahead of Unit 1. Unit 1, for an unknown reason, veered into the right lane from the left lane, and, in doing so, struck the left rear corner of Unit 2’s trailing unit. Unit 1 sustained damage to the front passenger’s side. Unit 2 sustained damage to its trailing unit. After impact, Unit 1 and Unit 2 both exited the roadway, where they came to their position of final rest on the right shoulder. Both Operator 1 and Operator 2 were wearing their seatbelts at the time of the crash. No airbags were deployed as a result of this crash. Operator 1 did not sustain injuries as a result of this crash. Operator 2 reported back pain and was evaluated on scene by medical personnel. Unit 1 was towed from the scene by Motter’s Towing. Unit 2 was driven from the scene by Operator 2. PSP Jonestown was assisted on scene by Life Lion Ambulance, Ft. Indiantown Gap Fire Department, and Motter’s Towing. Operator 1 was cited for Driving on Roadways Laned for Traffic.

Checkpoint DUI – On June 19, 2021 at 11:03 p.m., PSP Jonestown encountered a 63 year old Bernville man operating a vehicle through a DUI checkpoint on the 3,100 block of SR 72. The Operator was found to be under the influence of alcohol. Charges are to be filed. The Arrestee was a 63 year old Bernville man. The vehicles involved included a 2015 Ford Escape.
