Aging Care Manager 1 or 2

Starting Salary: $31,908.50 or $34,301.64

This Care Manager will serve as the primary information and referral person for the agency. This requires answering calls and assisting walk-in clients requesting information about agency services. This care manager will also carry a reduced case load. This would involve interviewing and evaluating consumers in their homes and developing an appropriate plan of care.

REQUIREMENTS (Aging Care Manager 1):

  • Must meet PA residency requirement.
  • Three years as an Aging Case Aide 2; OR
  • A bachelor’s degree which includes or is supplemented by twelve (12) credits in social sciences, behavioral sciences, human services; OR
  • An equivalent combination of experience and training.
  • Must be able to perform essential job functions.
  • Individuals hired as an Aging Care Manager 1 are promoted to Aging Care Manager 2 when eligible.

REQUIREMENTS (Aging Care Manager 2):

  • Must meet PA residency requirement.
  • Six months as an Aging Care Manager 1 or a County Caseworker 1; OR
  • Successful completion of the County Social Casework Intern program; OR
  • Six months in a professional human services or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; OR
  • An equivalent combination of experience and training.

Aging Care Management Supervisor 1

Starting Salary: $40,206.43 – Depends on Experience

Supervise all care management and casework activities with care managers in programs including OPTIONS, Person Centered Counseling (PCC), Care Giver Support (CSP) Program and Functional Eligibility Determination (FED).

Aging Care Manager 2, Protective Services

(Two positions available)

Starting Salary: $36,874.26 + $275/week periodic on-call pay

Evaluates the medical, social, and psychological need of older adults, investigates allegations of elder abuse, and offers services to detect, reduce, correct, or eliminate risks associated with all types of abuse identified during investigations. Work involves acting to ensure the well-being of older adults, as well as their living situation, financial circumstances, personal safety, and quality of life. Also takes referrals “on-call” after normal working hours, rotating weeks with other staff.

OPTIONS: Assesses needs and manages social services for older adults and their families to improve their quality of life. Work involves evaluating the social contacts and supports, daily living activities, cognitive functioning, physical health, and economic and other resources of older adults to develop plans for their care.  Includes coordinating plans with other agencies to refer clients for additional services and assisting older adults in utilizing available services to meet their needs.
